The clinicians are working alongside the company to help define physician-led advocacy as a new category. Together, they are shaping how software can extend expert guidance into the in-between moments that healthcare has historically failed to address outside traditional clinical encounters.

Why most healthcare decisions still happen outside clinical visits

CareCrowd is focused on a long-overlooked problem in healthcare known as the presence gap. It refers to the time after a visit, before the next encounter, when care is still unfolding and decisions still have to be made, but physicians are no longer present.

In these moments, patients and families are left to interpret information, manage fear, and act on uncertainty without expert guidance, even though the choices they make often shape outcomes.

“Here’s one thing we physicians haven’t said out loud as a profession,” said Dr. Mary Nwoke, Co-Founder of CareCrowd. “Just because we advocate informally does not mean we should leave it informal. Patients and families are making consequential decisions between visits every day. That advocacy already exists. It simply hasn’t been supported, structured, or made equitable.”

This behavior is not new. Physicians have always stepped in for family members, friends, and communities, helping to explain options, calm anxiety, and guide decisions outside formal encounters. When a doctor is involved, outcomes change. Yet tens of millions of Americans do not have access to a “doctor in the family,” leaving this gap unresolved for most people.

Despite how common this role is, physician-led advocacy has never been treated as a category. It has lacked shared language, enabling infrastructure, standards, and sustained investment. CareCrowd was formed around the belief that this gap should no longer be left to chance.

Building infrastructure for the moments between visits

CareCrowd is not focused on visits or transactions. It is focused on what happens after the visit, when real decisions still need to be made and no system exists to support them.

The company is building software to support physician-led advocacy in those moments, with the aim of bringing continuity and expert guidance into parts of healthcare that have long operated without either.

Early advocacy work is shaping what comes next

Early advocacy work is already happening in areas like elder care and cancer care, where physician advocates are helping patients and caregivers navigate difficult decisions between visits. This work is deliberately small and hands-on, focused on understanding how advocacy actually plays out in real situations before any attempt to scale it.

What emerges from these efforts is shaping how CareCrowd thinks about continuity, escalation, and support outside traditional care settings. At the same time, the company is working with a large patient and caregiver community of more than 300,000 people to help inform how this approach could evolve ahead of broader availability.

A founding bench of experienced physician advocates

The founding physician advisors come from a range of specialties and collectively bring decades of clinical and advocacy experience. Many already serve as informal advocates in their own families and communities, stepping in to explain options, calm concerns, and help people make sense of complex situations.

As part of CareCrowd, these physicians are helping ground the work in reality. Their role is to make sure anything built reflects how decisions actually happen outside the exam room, not how the system assumes they should.
